Narrative:
Item #1 (airspace violation) entered the class D airspace without first establishing 2-WAY radio communication. Reason: radio failure, saw no light gun signals. Suggestion: stronger light gun 'more visible' or a method of getting aircraft closer to tower within class D airspace to better observe. Light gun signals. Item #2 (runway incursion) failed to obtain a clearance to depart on active runway within class D airspace causing tower to divert aircraft on final approach. Reason: no radio, forgot no radio procedures. Suggestion: more visible light gun signals.

Title: RPTR ENTERS CLASS D AIRSPACE AND DEPARTS TWR CTL ARPT WITH INOP RADIOS AND WITHOUT GETTING LIGHT SIGNALS.
Narrative: ITEM #1 (AIRSPACE VIOLATION) ENTERED THE CLASS D AIRSPACE WITHOUT FIRST ESTABLISHING 2-WAY RADIO COM. REASON: RADIO FAILURE, SAW NO LIGHT GUN SIGNALS. SUGGESTION: STRONGER LIGHT GUN 'MORE VISIBLE' OR A METHOD OF GETTING ACFT CLOSER TO TWR WITHIN CLASS D AIRSPACE TO BETTER OBSERVE. LIGHT GUN SIGNALS.
