Narrative:
This is a late submission -- pending consult with ert member. Captain did not desire as soon as possible submission. On before starting engines checklist. Captain responded to FMS/mcdu item with brief of entries which included dallas 9 SID. I offered to hard tune dfw 090 degree radial as backup overlie of FMS course-captain declined. No xchk of flight plan/mcdu/map done. On departure -- ATC vectored flight to intercept and resume SID. Captain/PF intercepted displayed FMS cus in navigation. When established, ATC reported our return 5 mi left of course. My PNF map was on short range scale for traffic observation , but when expanded due to ATC report. I observed that SID was not displayed but captain had entered a course line dfw direct eic. Map data feature displayed us 9 mi left of course. This was easily preventable by adherence to SOP. Unfortunately -- this captain is not alone in failing to xchk flight plan/mcdu/map.
