Narrative:

Approach control gave altitude assignment to 4000 ft followed by a frequency change. Captain changed frequency and forgot to change altitude alert from 5000 to 4000 ft. When established contact on new approach frequency, captain read descending to 5000 ft, then 4000 ft. Controller then asked what altitude the previous controller had assigned us. Captain answered 4000 ft. Controller acknowledged and continued to vector us for the visual approach.
