Narrative:

Clt tower cleared for takeoff from runway 18R. We climbed to about 500 ft and saw off to the left a commuter (short) coming right at us. Immediately an evasive action was taken (stop climb and shallow bank right turn.) the commuter continued to climb and banked left. Controller while this was happening asked if we had the commuter -- then explained he forgot the commuter was there. Other controller had cleared the commuter for takeoff on runway 18L with a 270 degree heading after takeoff. The fact it was VFR, both aircraft were able to see and then take evasive action. We both (commuter jet) were situationally unaware because the tower frequencys for runway 18L/right are different. So, neither of us knew the other was cleared for takeoff. I fly in and out of clt often, the controllers do a great job. This was a rare mistake that fortunately did not end in disaster.

Title: NMAC -- TKOF.

Narrative: CLT TWR CLRED FOR TKOF FROM RWY 18R. WE CLBED TO ABOUT 500 FT AND SAW OFF TO THE L A COMMUTER (SHORT) COMING RIGHT AT US. IMMEDIATELY AN EVASIVE ACTION WAS TAKEN (STOP CLB AND SHALLOW BANK R TURN.) THE COMMUTER CONTINUED TO CLB AND BANKED L. CTLR WHILE THIS WAS HAPPENING ASKED IF WE HAD THE COMMUTER -- THEN EXPLAINED HE FORGOT THE COMMUTER WAS THERE. OTHER CTLR HAD CLRED THE COMMUTER FOR TKOF ON RWY 18L WITH A 270 DEG HDG AFTER TKOF. THE FACT IT WAS VFR, BOTH ACFT WERE ABLE TO SEE AND THEN TAKE EVASIVE ACTION. WE BOTH (COMMUTER JET) WERE SITUATIONALLY UNAWARE BECAUSE THE TWR FREQS FOR RWY 18L/R ARE DIFFERENT. SO, NEITHER OF US KNEW THE OTHER WAS CLRED FOR TKOF. I FLY IN AND OUT OF CLT OFTEN, THE CTLRS DO A GREAT JOB. THIS WAS A RARE MISTAKE THAT FORTUNATELY DID NOT END IN DISASTER.
