Narrative:
X was cleared for takeoff turned wbound and switched to departure control by myself working combined FD/ground control/local control with an in air emergency going on with local control/ground control. X did not tag up -- approach control lost track of flight and finally relocated them after they passed into sbn airspace. Our arw and FD in TRACON were combined at this time.

Narrative: X WAS CLRED FOR TKOF TURNED WBOUND AND SWITCHED TO DEP CTL BY MYSELF WORKING COMBINED FD/GC/LC WITH AN IN AIR EMER GOING ON WITH LC/GC. X DID NOT TAG UP -- APCH CTL LOST TRACK OF FLT AND FINALLY RELOCATED THEM AFTER THEY PASSED INTO SBN AIRSPACE. OUR ARW AND FD IN TRACON WERE COMBINED AT THIS TIME.
