Narrative:

Our flight was cleared for holding at stefe intersection at 11000 ft. Stefe is at ard 233 degrees and 23 DME. The first officer and myself both misidentified the fix to be at the 13 DME. We both referred to the nanci five STAR for lga. The low altitude chart is quite clear, but the STAR chart confused us both because it is not to scale. We were informed by center that we were 5 mi past the intersection, and to turn right immediately, because there were other flts in the area. Supplemental information from acn 298233: apparently holding was in progress at somto, as well.

Title: AN ACR FLC MISREAD A STAR INTXN'S LOCATION, AND CONTINUED PAST ASSIGNED HOLDING FIX. THE FLC WAS CONTINUING TOWARD, WHAT THEY PERCEIVED AS, THE ASSIGNED INTXN, WHEN ATC INTERVENED. BOTH OF THE INTXNS WERE IN USE, WITH HOLDING TFC.

WE BOTH REFERRED TO THE NANCI FIVE STAR FOR LGA. THE LOW ALT CHART IS QUITE CLR, BUT THE STAR CHART CONFUSED US BOTH BECAUSE IT IS NOT TO SCALE. WE WERE INFORMED BY CTR THAT WE WERE 5 MI PAST THE INTXN, AND TO TURN R IMMEDIATELY, BECAUSE THERE WERE OTHER FLTS IN THE AREA. SUPPLEMENTAL INFO FROM ACN 298233: APPARENTLY HOLDING WAS IN PROGRESS AT SOMTO, AS WELL.
