Narrative:
Working as a departure radar controller at bay TRACON, I was working runway 11 departures off of oak. Air carrier X departed on the wrong transponder code. ARTS data tag did not acquire on the target. During a high workload situation I didn't notice the lack of the data block and forgot air carrier X was on frequency and in my airspace. He entered center airspace without a handoff. Tower didn't advise me that the data block didn't acquire as required by the LOA and air carrier X on the wrong code plus my lack of awareness contributed to the error.

Narrative: WORKING AS A DEP RADAR CTLR AT BAY TRACON, I WAS WORKING RWY 11 DEPS OFF OF OAK. ACR X DEPARTED ON THE WRONG XPONDER CODE. ARTS DATA TAG DID NOT ACQUIRE ON THE TARGET.
