Narrative:

Mar/xx/95 aircraft flight xa btv to phl. ZNY gave clearance to cross 50 NM north of lvz VOR at FL180. Cruising at FL260. First officer flying aircraft. First officer entered an incorrect fix in the FMC. The fix was entered behind the aircraft. The error was discovered 4 NM from the actual fix. ARTCC was notified that we wouldn't make the restr. Center said basically 'no problem,' just continue to FL180. No traffic conflict was involved. I feel that the first officer incorrectly entered data in the FMC, failed to check the raw data available, and I, as PIC failed to double check the FMC data.

Title: FLC FAILED TO FLY AIRPLANE TO COMPLY WITH ALT RESTR.

Narrative: MAR/XX/95 ACFT FLT XA BTV TO PHL. ZNY GAVE CLRNC TO CROSS 50 NM N OF LVZ VOR AT FL180. CRUISING AT FL260. FO FLYING ACFT. FO ENTERED AN INCORRECT FIX IN THE FMC. THE FIX WAS ENTERED BEHIND THE ACFT. THE ERROR WAS DISCOVERED 4 NM FROM THE ACTUAL FIX. ARTCC WAS NOTIFIED THAT WE WOULDN'T MAKE THE RESTR. CTR SAID BASICALLY 'NO PROB,' JUST CONTINUE TO FL180. NO TFC CONFLICT WAS INVOLVED. I FEEL THAT THE FO INCORRECTLY ENTERED DATA IN THE FMC, FAILED TO CHK THE RAW DATA AVAILABLE, AND I, AS PIC FAILED TO DOUBLE CHK THE FMC DATA.
