Narrative:
At approximately 0x00z, small transport X was taxied into position and hold on runway 20C. Prior to reaching the runway local cleared X for takeoff and the aircraft departed from taxiway south (a parallel taxiway). This in itself wouldn't be too alarming. However, this is the 15TH to 20TH time this has happened since oct/xx/94. The airport authority/authorized is aware of situation is working to try and resolve problem. All types of pilots with a wide diverse experience from commercial pilot to student pilots, the taxiway was a former runway and is 150 ft wide, is easily confused. Callback conversation with reporter revealed the following information: reporter stated the facility management and airport management have been working on solving the problem. Latest is local controller cannot clear an aircraft for takeoff unless the controller assures the aircraft is on runway 20C. Bna tower supervisor stated FAA FSDO and ado are working with the city to resolve the problem. All airport markings and signage meet FAA standards. City has now put barricades across taxi at S6 to prevent further takeoff's from taxiway.

Narrative: AT APPROX 0X00Z, SMT X WAS TAXIED INTO POS AND HOLD ON RWY 20C. PRIOR TO REACHING THE RWY LCL CLRED X FOR TKOF AND THE ACFT DEPARTED FROM TXWY S (A PARALLEL TXWY). ALL TYPES OF PLTS WITH A WIDE DIVERSE EXPERIENCE FROM COMMERCIAL PLT TO STUDENT PLTS, THE TXWY WAS A FORMER RWY AND IS 150 FT WIDE, IS EASILY CONFUSED.
