Narrative:
Upon leveloff at FL250, the left bleed closed (right air conditioner pack was inoperative) causing aircraft to depressurize. An emergency was declared and descent was initiated and dispatch directed us to return to cvg. The rest of the flight was uneventful. The cause was a duct that came off, connecting bleed to pack.

Title: EMER DECLARED DSCNT PERFORMED AFTER ACR MDT LOSES CABIN PRESSURE. ACFT EQUIP PROB MALFUNCTION.
Narrative: UPON LEVELOFF AT FL250, THE L BLEED CLOSED (R AIR CONDITIONER PACK WAS INOP) CAUSING ACFT TO DEPRESSURIZE. AN EMER WAS DECLARED AND DSCNT WAS INITIATED AND DISPATCH DIRECTED US TO RETURN TO CVG. THE REST OF THE FLT WAS UNEVENTFUL. THE CAUSE WAS A DUCT THAT CAME OFF, CONNECTING BLEED TO PACK.
