Narrative:
Received clearance to 6000 ft by clearance delivery. A 5000 ft level off required by SID prior to departure contact. Nonstandard no handoff by tower to departure in use at airport. We climbed to 6000 ft, then contacted departure. Departure advised us that they controled airspace above 5000 ft. Contributing factors: 1) nonstandard handoff procedure. 2) SID plate has altitude information buried in corner under a different SID heading. 5 sids contained on 1 page. 3) fatigue a factor. 4) did not realize that tower did not handoff. 5) SID plate has information scattered.

Narrative: RECEIVED CLRNC TO 6000 FT BY CLRNC DELIVERY. A 5000 FT LEVEL OFF REQUIRED BY SID PRIOR TO DEP CONTACT. NONSTANDARD NO HDOF BY TWR TO DEP IN USE AT ARPT. WE CLBED TO 6000 FT, THEN CONTACTED DEP. DEP ADVISED US THAT THEY CTLED AIRSPACE ABOVE 5000 FT.
