Narrative:
Crew of 4: captain, first officer, flight attendant #1 seated at rear of aircraft, flight attendant #2 seated at forward of aircraft. Departed runway 4L. At approximately 600 ft AGL, 'master caution,' 'doors' and 'cabin' door warning lights illuminated. Felt aircraft depressurizing. Began immediate level off and power reduction to maintain as slow an airspeed as possible (flaps 15 degrees, climb sequence completed). Aircraft had intermittent yaw occilations. Instructed flight attendant (#1?) to 'stay away from the door.' notified ATC of emergency and we were returning to ord. Flight attendant #2 (seated in 1B as her jump seat was deferred) informed us that flight attendant #1 was being sucked out of aircraft, later said flight attendant #1 was gone. (Flight attendant #2 informed us still later that flight attendant #1 was in aircraft but injured and passenger helped pull her back into the aircraft.) informed ATC we wanted emergency equipment waiting. Landed runway 22R. (During flight and especially on final approach aircraft exhibited intermittent yaw oscillations.) after landing we requested equipment to follow us to 'penalty box' (aircraft holding area south of runways 22R and 9L) and we had injuries. Parked aircraft, sent first officer to cabin to check on flight attendant #1, flight attendant #2 and passenger. Called company, etc. Crash fire rescue equipment treated flight attendant #1 and transported to hospital. Flight attendant #2 treated for shock. Crash fire rescue equipment also informed us that door was not on aircraft. Passenger transported to terminal by bus.
