Narrative:

Amended release did not cover #1 EPR system discrepancy. We requested a new release for MEL item prior to departure. On ground before pushback agent handed us a new release. (This was for load permitting fuel). We departed without MEL item added to release. I was told we had the release. Also the EPR system appeared to work normal prior to departure.

Title: MEL ITEM NOT ON DISPATCH RELEASE.

Narrative: AMENDED RELEASE DID NOT COVER #1 EPR SYS DISCREPANCY. WE REQUESTED A NEW RELEASE FOR MEL ITEM PRIOR TO DEP. ON GND BEFORE PUSHBACK AGENT HANDED US A NEW RELEASE. (THIS WAS FOR LOAD PERMITTING FUEL). WE DEPARTED WITHOUT MEL ITEM ADDED TO RELEASE. I WAS TOLD WE HAD THE RELEASE. ALSO THE EPR SYS APPEARED TO WORK NORMAL PRIOR TO DEP.
