Narrative:
This is in response to investigation of upsets. In the mid 70's we had a 141 roll over at 41000 ft, went supersonic in the dive and pulled out at 12000 ft. I believe it was attributed to a bad yaw damper. Aircraft and pilots were in my squadron at xyz AFB.

Title: MIL FLC IN A C141 EXPERIENCES LOSS OF ACFT CTL.
Narrative: THIS IS IN RESPONSE TO INVESTIGATION OF UPSETS. IN THE MID 70'S WE HAD A 141 ROLL OVER AT 41000 FT, WENT SUPERSONIC IN THE DIVE AND PULLED OUT AT 12000 FT. I BELIEVE IT WAS ATTRIBUTED TO A BAD YAW DAMPER. ACFT AND PLTS WERE IN MY SQUADRON AT XYZ AFB.
