Narrative:

#4 engine failure on aircraft oct/xx/95 at mia. Flight had a normal 4 engine start and taxi to runway 9L at mia. After receiving takeoff clearance from miami tower, captain positioned the aircraft on the centerline of runway 9L, completed the takeoff checklist and set the parking brake. I, the first officer making the takeoff, then positioned the thrust levers to the vertical position. All 4 engine indications appeared stabilized and normal. I then advanced the thrust levers toward the target EPR that was set for takeoff while saying 'set takeoff thrust.' the takeoff roll was normal with all engines operating normally and aircraft acceleration normal. At V1 I heard a loud bang and noticed a slight yaw which I immediately corrected with rudder. Captain simultaneously and instantly recognized the total failure of the #4 engine and responded by immediately assuming control of the aircraft and began to accomplish the rejected takeoff procedure. My crew member duties immediately shifted from PF to PNF. I then began performing the duties of a first officer for a rejected takeoff. After I was assured that the aircraft would stop on the remaining runway available and my assistance to the captain was minimal, I picked up the microphone and called mia tower. I stated 'miami tower, aircraft has rejected the takeoff and we are still on the runway.' the tower responded 'roger still on the runway.' captain stopped the aircraft with approximately 1000 ft of runway remaining on runway 9L and then set the parking brake. At that time mia tower called to advise us that it appeared our #4 engine was on fire. We acknowledged and captain called for the engine fire, severe damage/separation checklist to be accomplished. After initiating this checklist the mia tower advised us that our #4 engine was still on fire. We had no fire indication in the cockpit. Captain then directed the discharge of the second fire bottle to the #4 engine. The mia fire/emergency team arrived and extinguished any and all remaining flames on engine #4. The flight crew remained on the aircraft to complete all appropriate checklists and be in position for subsequent towing of the aircraft by ground personnel. This report of the events that occurred at XA50 am on the morning of oct/xx/95 is true and correct to the best of my knowledge. I would like to commend the personnel in the control tower for their assistance and professional support. Special thanks and recognition should be given to the mia fire/emergency team for their rapid response during this incident.

Title: ABORTED TKOF BECAUSE OF ENG FAILURE.

Narrative: #4 ENG FAILURE ON ACFT OCT/XX/95 AT MIA. FLT HAD A NORMAL 4 ENG START AND TAXI TO RWY 9L AT MIA. AFTER RECEIVING TKOF CLRNC FROM MIAMI TWR, CAPT POSITIONED THE ACFT ON THE CTRLINE OF RWY 9L, COMPLETED THE TKOF CHKLIST AND SET THE PARKING BRAKE. I, THE FO MAKING THE TKOF, THEN POSITIONED THE THRUST LEVERS TO THE VERT POS. ALL 4 ENG INDICATIONS APPEARED STABILIZED AND NORMAL. I THEN ADVANCED THE THRUST LEVERS TOWARD THE TARGET EPR THAT WAS SET FOR TKOF WHILE SAYING 'SET TKOF THRUST.' THE TKOF ROLL WAS NORMAL WITH ALL ENGS OPERATING NORMALLY AND ACFT ACCELERATION NORMAL. AT V1 I HEARD A LOUD BANG AND NOTICED A SLIGHT YAW WHICH I IMMEDIATELY CORRECTED WITH RUDDER. CAPT SIMULTANEOUSLY AND INSTANTLY RECOGNIZED THE TOTAL FAILURE OF THE #4 ENG AND RESPONDED BY IMMEDIATELY ASSUMING CTL OF THE ACFT AND BEGAN TO ACCOMPLISH THE REJECTED TKOF PROC. MY CREW MEMBER DUTIES IMMEDIATELY SHIFTED FROM PF TO PNF. I THEN BEGAN PERFORMING THE DUTIES OF A FO FOR A REJECTED TKOF. AFTER I WAS ASSURED THAT THE ACFT WOULD STOP ON THE REMAINING RWY AVAILABLE AND MY ASSISTANCE TO THE CAPT WAS MINIMAL, I PICKED UP THE MIKE AND CALLED MIA TWR. I STATED 'MIAMI TWR, ACFT HAS REJECTED THE TKOF AND WE ARE STILL ON THE RWY.' THE TWR RESPONDED 'ROGER STILL ON THE RWY.' CAPT STOPPED THE ACFT WITH APPROX 1000 FT OF RWY REMAINING ON RWY 9L AND THEN SET THE PARKING BRAKE. AT THAT TIME MIA TWR CALLED TO ADVISE US THAT IT APPEARED OUR #4 ENG WAS ON FIRE. WE ACKNOWLEDGED AND CAPT CALLED FOR THE ENG FIRE, SEVERE DAMAGE/SEPARATION CHKLIST TO BE ACCOMPLISHED. AFTER INITIATING THIS CHKLIST THE MIA TWR ADVISED US THAT OUR #4 ENG WAS STILL ON FIRE. WE HAD NO FIRE INDICATION IN THE COCKPIT. CAPT THEN DIRECTED THE DISCHARGE OF THE SECOND FIRE BOTTLE TO THE #4 ENG. THE MIA FIRE/EMER TEAM ARRIVED AND EXTINGUISHED ANY AND ALL REMAINING FLAMES ON ENG #4. THE FLC REMAINED ON THE ACFT TO COMPLETE ALL APPROPRIATE CHKLISTS AND BE IN POS FOR SUBSEQUENT TOWING OF THE ACFT BY GND PERSONNEL. THIS RPT OF THE EVENTS THAT OCCURRED AT XA50 AM ON THE MORNING OF OCT/XX/95 IS TRUE AND CORRECT TO THE BEST OF MY KNOWLEDGE. I WOULD LIKE TO COMMEND THE PERSONNEL IN THE CTL TWR FOR THEIR ASSISTANCE AND PROFESSIONAL SUPPORT. SPECIAL THANKS AND RECOGNITION SHOULD BE GIVEN TO THE MIA FIRE/EMER TEAM FOR THEIR RAPID RESPONSE DURING THIS INCIDENT.
