Narrative:
A cherokee was instructed to taxi into position and hold on runway 11L. An air carrier rolled out on runway 11L, was issued exiting instructions. After the aircraft exited, the cherokee was cleared for takeoff on runway 11L. A DV20, who landed on runway 11R, was instructed to make a 180 degree and taxi back via runway 11R. I was attending to other position duties when I looked up to realize the cherokee had taxied off runway 11L, departed runway 11R and was nearing rotation speed. I advised the DV20 of the cherokee, the DV20 steered to the right side of runway 11R, the cherokee rotated approximately 100 ft before DV20.

Title: FOREIGN REGISTERED PA28 CLRED FOR TKOF ON RWY 11L, TOOK OFF ON RWY 11R HEAD ON TOWARD A DV20 THAT HAD JUST LANDED AND HAD BEEN CLRED FOR A 180 DEG ON THE RWY. LCL CTLR NOTICED THE PROB IN TIME TO WARN THE DV20, WHO PULLED OFF TO THE R SIDE OF THE RWY AS THE PA28 BROKE GND IN FRONT OF HIM.
Narrative: A CHEROKEE WAS INSTRUCTED TO TAXI INTO POS AND HOLD ON RWY 11L. AN ACR ROLLED OUT ON RWY 11L, WAS ISSUED EXITING INSTRUCTIONS. AFTER THE ACFT EXITED, THE CHEROKEE WAS CLRED FOR TKOF ON RWY 11L. A DV20, WHO LANDED ON RWY 11R, WAS INSTRUCTED TO MAKE A 180 DEG AND TAXI BACK VIA RWY 11R. I WAS ATTENDING TO OTHER POS DUTIES WHEN I LOOKED UP TO REALIZE THE CHEROKEE HAD TAXIED OFF RWY 11L, DEPARTED RWY 11R AND WAS NEARING ROTATION SPD. I ADVISED THE DV20 OF THE CHEROKEE, THE DV20 STEERED TO THE R SIDE OF RWY 11R, THE CHEROKEE ROTATED APPROX 100 FT BEFORE DV20.
