Narrative:

Flew with student for flight test to ile, tx, from F60 then back to F60. After, designated pilot examiner advised us that weight and balance paperwork was not documented correctly. Our a&P had reweighed aircraft and recomputed center of gravity after approximately 120 pounds of avionics were removed in january of this yr. Aircraft was out of center of gravity so ballasts were added to put center of gravity back into envelope. However, a&P didn't document the use of ballasts to correct center of gravity nor did he sign new weight and balance.
