Narrative:
On the ramp at ord the deicer checked in. I specifically asked that type 1 and type 2 fluid be used on the wings and the tail. After deicing, deicer reported wings and tail deiced with type 1 and type 2. I told him the tail had not been deiced with type 2 -- I was observing. He then deiced tail with type 2. Ramp crew came out to aircraft, checked in on headset, and reported that they were ready to push back when we were ready. I asked if post-deicing check had been performed. Crew did not know. I asked that the crew chief come out to aircraft. Crew chief came to aircraft and then performed post deicing check. If I had not asked about the post deicing check, one would not have been done.

Title: MD80 FLC EXPERIENCED FRUSTRATION WITH GND CREW DURING DEICING AND ANTI-ICING PROCS. THE GND CREW FAILED TO FOLLOW THE CREW MEMBER'S SPECS REGARDING TYPES OF FLUID AND DID NOT COORDINATE PRETKOF CONTAMINATION CHK.
Narrative: ON THE RAMP AT ORD THE DEICER CHKED IN. I SPECIFICALLY ASKED THAT TYPE 1 AND TYPE 2 FLUID BE USED ON THE WINGS AND THE TAIL. AFTER DEICING, DEICER RPTED WINGS AND TAIL DEICED WITH TYPE 1 AND TYPE 2. I TOLD HIM THE TAIL HAD NOT BEEN DEICED WITH TYPE 2 -- I WAS OBSERVING. HE THEN DEICED TAIL WITH TYPE 2. RAMP CREW CAME OUT TO ACFT, CHKED IN ON HEADSET, AND RPTED THAT THEY WERE READY TO PUSH BACK WHEN WE WERE READY. I ASKED IF POST-DEICING CHK HAD BEEN PERFORMED. CREW DID NOT KNOW. I ASKED THAT THE CREW CHIEF COME OUT TO ACFT. CREW CHIEF CAME TO ACFT AND THEN PERFORMED POST DEICING CHK. IF I HAD NOT ASKED ABOUT THE POST DEICING CHK, ONE WOULD NOT HAVE BEEN DONE.
