Narrative:

ATC delay at gate sfo due to thunderstorm activity destination (lax). First officer called clearance delivery who told him contact ground for length of delay. First officer forgot to get clearance. We took off without our clearance. First indication of a problem when aircraft started to level at 2500 ft. FMC set to 2500 ft not FL230. Second, squawk was incorrect. Third, departure said maintain 250 KTS and I said slowing to 250 from 300 KTS. ATC asked if we had been given normal speed. First officer has 80 hours on aircraft. It was his first takeoff with me (first day of 2 day trip). I had not flown in 17 days and only 130 hours in type in last 6 months. It never occurred to me first officer would forget to get clrncs.
