Narrative:

During a routine 'quick turn,' in order for the flight to depart on time, the captain went to pick up the current WX report and I went to the bathroom. Supplemental information from acn 333341: we departed from lax to ipl with insufficient fuel for the trip. We were on a 15 min turn and both first officer and I thought the other had ordered fuel. Neither of us did. When we came to fuel check on after start checklist, this item was somehow missed. Just after takeoff I noticed we had not received fuel in lax. I requested to return to lax. We put on more fuel and then departed 40 mins late. I notified dispatch and assistant chief pilot of the incident. Contributing factors: both first officer and I saw a fuel truck in front of our aircraft and we assumed it was being fueled. First officer even asked ramper if we had gotten fuel, his response was 'yes.' this was a quick turn and we were rushed. We were unable to remain at aircraft while fueling was done due to physical needs and checking WX. Only place there is a check is one time on after start checklist. Possibly there needs to be another place later down the line. We were both in the mindset that we had received fuel due to earlier observations which may have caused complacency. Our checklist call is 'fuel quantity/balance -- checked' maybe rather than just 'checked' as a response an actual statement of amount of fuel on the gauges would cause closer look at gauges.

OUR CHKLIST CALL IS 'FUEL QUANTITY/BAL -- CHKED' MAYBE RATHER THAN JUST 'CHKED' AS A RESPONSE AN ACTUAL STATEMENT OF AMOUNT OF FUEL ON THE GAUGES WOULD CAUSE CLOSER LOOK AT GAUGES.
