Narrative:
Took off without ATC route clearance. Rushed departure duties due to fuel change at last min. Thunderstorms were approaching destination airport (tul) and that was the reason for the additional fuel. The missed approach altitude (5000 ft) was set in the altitude window which is also the standard pit departure altitude.

Title: FLT DEPARTED WITHOUT IFR CLRNC. RUSHED DEP AND LAST MIN FUEL CHANGE, DUE TO WX, CONTRIBUTED TO FLC FORGETTING TO GET CLRNC.
Narrative: TOOK OFF WITHOUT ATC RTE CLRNC. RUSHED DEP DUTIES DUE TO FUEL CHANGE AT LAST MIN. TSTMS WERE APCHING DEST ARPT (TUL) AND THAT WAS THE REASON FOR THE ADDITIONAL FUEL. THE MISSED APCH ALT (5000 FT) WAS SET IN THE ALT WINDOW WHICH IS ALSO THE STANDARD PIT DEP ALT.
