Narrative:
Center told us to cross srptn intersection +50 west at 210 degrees, at 53 mi west they told us again. As the captain, I was responsible, but my headphone jack had come out and I was unaware that we had been given a descent. I am very unhappy about this as our standard operating procedures would have caught this if my first officer had called the change in altitude. The fact that there is no side tone on this aircraft did not help. I know that this sounds like I am whining to get an out of jail free card, but I am just tired of training all these new guys! I have to put my ticket on the line constantly account of these trainees keep making the same mistakes over and over again! Who is teaching this new group of airmen? Also, 10 hour rest and 14 hour days don't help either.
