Narrative:
The airline I fly for has just begun using printed pre departure clearance's brought to the cockpit just before pushback (see attached). We were departing lax with the enclosed pre departure clearance. I read this as laxx 2 departure dag transition flight planned route. On departure we were initially restr to 2000 ft and then 5000 ft on a vector. Finally we were told to resume the loop 9 departure, climb and maintain 15000 ft. We advised ATC that we were on the laxx 2. After a few moments he advised us that we were cleared the loop 9 per tower and could we make the altitude restrs? We were heavy and advised him we could not make the 13000 ft at 8 DME lax 041 degree radial, but we did make 15000 ft at 20 DME. ATC advised us it would not be a problem, he would provide any necessary vertical separation and there were no conflicts. After reviewing the pre departure clearance, we saw the loop 9 was indicated, but the laxx 2 was also indicated along with the routing. We focused on the laxx 2 and completely missed the loop 9 along with the effective times of the departures. To avoid confusion, pre departure clearance should only list the cleared routing.

Title: PRE DEP CLRNC MISINTERP. RPTR'S COMPANY RECENTLY BECAME USERS OF THE PDC FOR FLCS. THE RPTR FLC DIDN'T UNDERSTAND WHAT THE 'LOOP 9 DAG' ADDITION MEANT AT THE TOP OF THE PDC. THAT INDICATED A REVISION WHICH THEY DIDN'T KNOW. THUS, THEY STARTED THE DEP ON A DIFFERENT SID UNTIL ADVISED BY ATC OF THE CORRECT ROUTING.
