Narrative:

During climb out flight attendant came to cockpit to ask if there was a reason why some oxygen masks were out. She then indicated all masks were out. There were no annunciator cautions or warnings (should have been two). Almost simultaneously, while rechking system, flight attendant returned and said people were having problems, cabin altitude indicated 18000 ft and forward cargo door caution light came on. Requested immediate descent, directed passenger to don oxygen masks over PA, initiated emergency descent, diverted to dca and landed. Medical personnel met aircraft at gate. Cabin low pressure warning horn should be installed for all pressure controllers not just dual.

Title: AN MLG COMMERCIAL ACFT ON CLBOUT DECLARED AN EMER AND DIVERTED DUE TO LOSS OF PRESSURIZATION CAUSED BY THE FORWARD CARGO DOOR NOT BEING CLOSED AND LOCKED.

Narrative: DURING CLBOUT FLT ATTENDANT CAME TO COCKPIT TO ASK IF THERE WAS A REASON WHY SOME OXYGEN MASKS WERE OUT. SHE THEN INDICATED ALL MASKS WERE OUT. THERE WERE NO ANNUNCIATOR CAUTIONS OR WARNINGS (SHOULD HAVE BEEN TWO). ALMOST SIMULTANEOUSLY, WHILE RECHKING SYS, FLT ATTENDANT RETURNED AND SAID PEOPLE WERE HAVING PROBS, CABIN ALT INDICATED 18000 FT AND FORWARD CARGO DOOR CAUTION LIGHT CAME ON. REQUESTED IMMEDIATE DSCNT, DIRECTED PAX TO DON OXYGEN MASKS OVER PA, INITIATED EMER DSCNT, DIVERTED TO DCA AND LANDED. MEDICAL PERSONNEL MET ACFT AT GATE. CABIN LOW PRESSURE WARNING HORN SHOULD BE INSTALLED FOR ALL PRESSURE CTLRS NOT JUST DUAL.
