Narrative:
Center gear pin failed to be removed before flight. Aircraft xx date sep/zz/97 iah-ewr, no log page entry, center pin installed, no flag or streamer visible from ground. In air, center gear failed to retract. Control center asked crew how many gear pins on board -- 3 as per form for dc-10 normal checklist gear pins. Onboard crew failed to count 4. Mechanic that removed gear pins failed to count in pouch the fourth pin. Suggest separate write-up when center gear pin is installed on DC10-30.

Narrative: CTR GEAR PIN FAILED TO BE REMOVED BEFORE FLT. ACFT XX DATE SEP/ZZ/97 IAH-EWR, NO LOG PAGE ENTRY, CTR PIN INSTALLED, NO FLAG OR STREAMER VISIBLE FROM GND.
