Narrative:
I believe the design of the prado five departure out of ont on the commercial page caused a possible altitude deviation due to the location of the 4000 ft restr at 6 DME of pdz. The note on the page is located way to the right of the 6 NM fix and can be easily overlooked, as I did. I believe it needs to be located nearer to the fix on the left side of the page.

Title: FLC OF AN MLG OVERSHOT PUBLISHED SID DEP ALT DURING CLB AFTER TKOF. RPTR BELIEVES THAT THE NOTE FOR THE ALT RESTR IS NOT CLOSE ENOUGH TO THE FIX TO BE EASILY OBSERVED.
Narrative: I BELIEVE THE DESIGN OF THE PRADO FIVE DEP OUT OF ONT ON THE COMMERCIAL PAGE CAUSED A POSSIBLE ALTDEV DUE TO THE LOCATION OF THE 4000 FT RESTR AT 6 DME OF PDZ. THE NOTE ON THE PAGE IS LOCATED WAY TO THE R OF THE 6 NM FIX AND CAN BE EASILY OVERLOOKED, AS I DID. I BELIEVE IT NEEDS TO BE LOCATED NEARER TO THE FIX ON THE L SIDE OF THE PAGE.
