Narrative:

On the last day of pairing, considerable fatigue was apparent. During the leg from rno to phx, both of us, the pilots, felt unsafe. I was the PNF in charge of radio communications. I missed numerous frequency changes and at one point accepted a clearance meant for another flight number (my phone number). On short final, I mistakenly turned off both ILS displays thinking runway 26R was runway 26L. With each other's help we managed to complete the flight without incident. This type of schedule should be illegal. Airlines should not be allowed to fly their pilots up to maximum time permitted.

Narrative: ON THE LAST DAY OF PAIRING, CONSIDERABLE FATIGUE WAS APPARENT. DURING THE LEG FROM RNO TO PHX, BOTH OF US, THE PLTS, FELT UNSAFE. I WAS THE PNF IN CHARGE OF RADIO COMS. ON SHORT FINAL, I MISTAKENLY TURNED OFF BOTH ILS DISPLAYS THINKING RWY 26R WAS RWY 26L. WITH EACH OTHER'S HELP WE MANAGED TO COMPLETE THE FLT WITHOUT INCIDENT. THIS TYPE OF SCHEDULE SHOULD BE ILLEGAL. AIRLINES SHOULD NOT BE ALLOWED TO FLY THEIR PLTS UP TO MAX TIME PERMITTED.
