Narrative:
A lavatory was put on MEL. This MEL item was not included on the flight release. Contributing factors: item was deferred in flight attendant logbook. Workload, as other maintenance and deferrals were being accomplished. In the aircraft/cockpit logbook the transmission of the deferral from the flight attendant logbook to dispatch was forgotten by the pilot (me) and was not timely by maintenance. Having only one logbook for the aircraft might have helped this error.

Title: AN MD80 WAS DISPATCHED WITH AN INOP LAVATORY DEFERRED PER THE MEL BUT NOT ENTERED ON THE FLT RELEASE DOCUMENT.
Narrative: A LAVATORY WAS PUT ON MEL. THIS MEL ITEM WAS NOT INCLUDED ON THE FLT RELEASE. CONTRIBUTING FACTORS: ITEM WAS DEFERRED IN FLT ATTENDANT LOGBOOK. WORKLOAD, AS OTHER MAINT AND DEFERRALS WERE BEING ACCOMPLISHED. IN THE ACFT/COCKPIT LOGBOOK THE XMISSION OF THE DEFERRAL FROM THE FLT ATTENDANT LOGBOOK TO DISPATCH WAS FORGOTTEN BY THE PLT (ME) AND WAS NOT TIMELY BY MAINT. HAVING ONLY ONE LOGBOOK FOR THE ACFT MIGHT HAVE HELPED THIS ERROR.
