Narrative:
How the problem arose: when I called tower, stating ready for takeoff on runway 19R. The tower stated cancel runway 19R, aircraft on 2 mi final, cleared for runway 19L. I looked at my flight guide and saw one taxiway before runway 19L. There were no aircraft at all in the area around me. I crossed past the center taxiway and was prepared and cleared for takeoff on runway 19L. I came upon what I assumed would be runway 19L. (No sign stating taxiway as per first taxiway in center.) I started ground roll with power. Contributing factors: first time into las vegas mccarran international airport. Prepared for runway 19R. Flight guide 6 weeks old purchase. Diagram of runway 19R plus diagram of txwys in center wrong. 3A -- shown runway 19R length as 5001 ft. 3B -- shows only 1 taxiway in center between runways. Tower might have been able to alert me. Could not see any sign on east center taxiway at runway 19R or runway 19L. How it was discovered: I discovered that as I moved over the ground there was no white centerline, but there was a remaining yellow taxi line. Corrective actions: my reaction to this was to scan for any obstacles or aircraft on this pavement. Get off the ground as soon as possible and contact the tower to alert them of the problem at hand. Tower stated a phone number to call as soon as I reached my destination. I did so. Perceptions, judgements, and decisions: while taxiing my wife stated that the other aircraft had taken off at this intersection a few moments earlier. Factors affecting the quality of human performance: I did not see other aircraft depart because I was looking at my flight guide at the time. I took into consideration what was said by my wife about the departed aircraft. Actions or inactions: my actions were based primarily on the diagram of runways and txwys shown in the flight guide airport and frequency manual. Callback conversation with reporter revealed the following information: reporter states that he now believes he was mistaken about there being 2 txwys between the runways. He even phoned the tower directly to determine if there are 2 txwys and was informed there is just 1 taxiway between the parallel runways. This was a daytime operation so the lack of awareness cannot be accounted for from a visibility standpoint. The only major difference between the current airport diagram and the expired one is the length of runway 19R which has been extended. Reporter is quite chagrined.

Title: PLT MAKES HIS TKOF ON A TXWY BTWN THE PARALLEL RWYS BELIEVING HE HAS ALREADY CROSSED THE TXWY.
Narrative: HOW THE PROB AROSE: WHEN I CALLED TWR, STATING READY FOR TKOF ON RWY 19R. THE TWR STATED CANCEL RWY 19R, ACFT ON 2 MI FINAL, CLRED FOR RWY 19L. I LOOKED AT MY FLT GUIDE AND SAW ONE TXWY BEFORE RWY 19L. THERE WERE NO ACFT AT ALL IN THE AREA AROUND ME. I CROSSED PAST THE CTR TXWY AND WAS PREPARED AND CLRED FOR TKOF ON RWY 19L. I CAME UPON WHAT I ASSUMED WOULD BE RWY 19L. (NO SIGN STATING TXWY AS PER FIRST TXWY IN CTR.) I STARTED GND ROLL WITH PWR. CONTRIBUTING FACTORS: FIRST TIME INTO LAS VEGAS MCCARRAN INTL ARPT. PREPARED FOR RWY 19R. FLT GUIDE 6 WKS OLD PURCHASE. DIAGRAM OF RWY 19R PLUS DIAGRAM OF TXWYS IN CTR WRONG. 3A -- SHOWN RWY 19R LENGTH AS 5001 FT. 3B -- SHOWS ONLY 1 TXWY IN CTR BTWN RWYS. TWR MIGHT HAVE BEEN ABLE TO ALERT ME. COULD NOT SEE ANY SIGN ON E CTR TXWY AT RWY 19R OR RWY 19L. HOW IT WAS DISCOVERED: I DISCOVERED THAT AS I MOVED OVER THE GND THERE WAS NO WHITE CTRLINE, BUT THERE WAS A REMAINING YELLOW TAXI LINE. CORRECTIVE ACTIONS: MY REACTION TO THIS WAS TO SCAN FOR ANY OBSTACLES OR ACFT ON THIS PAVEMENT. GET OFF THE GND AS SOON AS POSSIBLE AND CONTACT THE TWR TO ALERT THEM OF THE PROB AT HAND. TWR STATED A PHONE NUMBER TO CALL AS SOON AS I REACHED MY DEST. I DID SO. PERCEPTIONS, JUDGEMENTS, AND DECISIONS: WHILE TAXIING MY WIFE STATED THAT THE OTHER ACFT HAD TAKEN OFF AT THIS INTXN A FEW MOMENTS EARLIER. FACTORS AFFECTING THE QUALITY OF HUMAN PERFORMANCE: I DID NOT SEE OTHER ACFT DEPART BECAUSE I WAS LOOKING AT MY FLT GUIDE AT THE TIME. I TOOK INTO CONSIDERATION WHAT WAS SAID BY MY WIFE ABOUT THE DEPARTED ACFT. ACTIONS OR INACTIONS: MY ACTIONS WERE BASED PRIMARILY ON THE DIAGRAM OF RWYS AND TXWYS SHOWN IN THE FLT GUIDE ARPT AND FREQ MANUAL. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: RPTR STATES THAT HE NOW BELIEVES HE WAS MISTAKEN ABOUT THERE BEING 2 TXWYS BTWN THE RWYS. HE EVEN PHONED THE TWR DIRECTLY TO DETERMINE IF THERE ARE 2 TXWYS AND WAS INFORMED THERE IS JUST 1 TXWY BTWN THE PARALLEL RWYS. THIS WAS A DAYTIME OP SO THE LACK OF AWARENESS CANNOT BE ACCOUNTED FOR FROM A VISIBILITY STANDPOINT. THE ONLY MAJOR DIFFERENCE BTWN THE CURRENT ARPT DIAGRAM AND THE EXPIRED ONE IS THE LENGTH OF RWY 19R WHICH HAS BEEN EXTENDED. RPTR IS QUITE CHAGRINED.
