Narrative:
Our flight was at FL210 on the mizar two arrival into dtw over the fwa VOR. I was copying the dtw ATIS from the ACARS display on the air carrier's ATIS form, while my first officer was flying the airplane. ZAU asked if we were on the mizar two arrival. I responded affirmative as I was checking the HSI. ATC said that she showed us 5 mi north of course. I saw that we were 8 DME from the fwa VOR on the 360 degree radial. My first officer responded to the error by turning the airplane eastward, while at the same time ATC instructed us to turn right to 090 degrees to intercept the fwa 039 degree radial, and continue the mizar two. My first officer said that he misread the commercial chart, and put in the radial off of dqn in the HSI, instead of fwa.
