Narrative:

We were cleared for the visual approach to runway 21 at gsp about 5 mi out on a left base. Upon landing approach frequency was still up on the communication in use, so we contacted tower. He said he tried to call us and cleared us to taxi to the ramp. So we called tower and he advised us that approach forgot to switch us over to tower. The contributing factors were: fatigue from a long day/week and we did not catch the mistake early enough. While conducting landing checks, be more observant during landing phase. Supplemental information from acn 404718: I was sure that the last controller cleared us to land and never told us to go to the tower frequency.
