Narrative:
While on the ground at the jet center, I watched the fuel boy top off both tanks. I checked for water and then went for takeoff. As soon as airborne, the left wing fuel cap came off and, coming back to land, we lost 17 gals of fuel. I showed the fuel boy who was waiting how it happened, but they denied any responsibility.

Title: SINGLE ENG ACFT LOSES FUEL CAP ON TKOF AT RVS.
Narrative: WHILE ON THE GND AT THE JET CTR, I WATCHED THE FUEL BOY TOP OFF BOTH TANKS. I CHKED FOR WATER AND THEN WENT FOR TKOF. AS SOON AS AIRBORNE, THE L WING FUEL CAP CAME OFF AND, COMING BACK TO LAND, WE LOST 17 GALS OF FUEL. I SHOWED THE FUEL BOY WHO WAS WAITING HOW IT HAPPENED, BUT THEY DENIED ANY RESPONSIBILITY.
