Narrative:
Started to taxi without clearance. Controller said 'oh, I guess you guys are ready to taxi.' last day of 4 day trip, XA30 wakeup, XB00 bus, XC05 takeoff. Flight attendant entered cockpit at taxi time to ask if we had animals in bin #1 (cargo hold #1). We checked paperwork. At the same time our ramp agent was giving us taxi signals after our 'goodbye salute.' we were distraction by flight attendant and sucked in by the hand signals and tired. Our habit pattern was broken.
