Narrative:

This happened on feb/xa/98, just now informed. I do not recall this problem. I followed the required inspection item book, and as a last check I checked to see the pin and cable was hooked up, pin removed. On a routine deployment of slide 4L, FAA and maintenance found it would not deploy. They found the pins installed and cable not hooked up, pins taped to side.

Title: A B747-400 WAS DISPATCHED WITH A #4 L DOOR ESCAPE SLIDE IMPROPERLY INSTALLED PREVENTING ESCAPE SLIDE DEPLOYMENT.

Narrative: THIS HAPPENED ON FEB/XA/98, JUST NOW INFORMED. I DO NOT RECALL THIS PROB. I FOLLOWED THE REQUIRED INSPECTION ITEM BOOK, AND AS A LAST CHK I CHKED TO SEE THE PIN AND CABLE WAS HOOKED UP, PIN REMOVED. ON A ROUTINE DEPLOYMENT OF SLIDE 4L, FAA AND MAINT FOUND IT WOULD NOT DEPLOY. THEY FOUND THE PINS INSTALLED AND CABLE NOT HOOKED UP, PINS TAPED TO SIDE.
