Narrative:
On aug/xy/98, my crew chief came to me about XX00 and told me we had a problem with pumps. It was brought to my attention that there were 2 pumps, a 4 and 5 that the nut on the drive gear was neither tightened nor safetied with a cotter pin. I then checked the date they were sent out. I checked my log and realized that I had sent out 4 pumps on that day. I then went and found the other 2 pumps and called them to their attention. I am sorry for what has happened. I absolutely did not do this on purpose. It happened on the day before I was going on vacation. I was trying to get these pumps done before I left, so the engine shop would be covered while I was gone on vacation. Therefore, I got in a hurry and forgot to tighten and put cotter pins in all 4 pumps.

Title: A TOTAL OF 4 JT8D-209 ENG OIL SCAVENGE PUMPS WERE ASSEMBLED INCORRECTLY WITHOUT THE DRIVE GEAR NUT TORQUED OR COTTER PIN INSTALLED.
Narrative: ON AUG/XY/98, MY CREW CHIEF CAME TO ME ABOUT XX00 AND TOLD ME WE HAD A PROB WITH PUMPS. IT WAS BROUGHT TO MY ATTN THAT THERE WERE 2 PUMPS, A 4 AND 5 THAT THE NUT ON THE DRIVE GEAR WAS NEITHER TIGHTENED NOR SAFETIED WITH A COTTER PIN. I THEN CHKED THE DATE THEY WERE SENT OUT. I CHKED MY LOG AND REALIZED THAT I HAD SENT OUT 4 PUMPS ON THAT DAY. I THEN WENT AND FOUND THE OTHER 2 PUMPS AND CALLED THEM TO THEIR ATTN. I AM SORRY FOR WHAT HAS HAPPENED. I ABSOLUTELY DID NOT DO THIS ON PURPOSE. IT HAPPENED ON THE DAY BEFORE I WAS GOING ON VACATION. I WAS TRYING TO GET THESE PUMPS DONE BEFORE I LEFT, SO THE ENG SHOP WOULD BE COVERED WHILE I WAS GONE ON VACATION. THEREFORE, I GOT IN A HURRY AND FORGOT TO TIGHTEN AND PUT COTTER PINS IN ALL 4 PUMPS.
