Narrative:
IFR flight plan to cincinnati area. Had plate out for hamilton airport, localizer approach, but localizer OTS. Received authority/authorized for NDB approach vectors. Could not produce plates in time, lost in cockpit. Missed approach. ATC vectored to 2600 ft minimum but unable to see airport. Received vectors to back course approach at luk, but missed. Messed up approach again due to failure to produce plates from cockpit. Executed missed approach and received vectors for ILS to opposite end of same runway. No further difficulties. Landed without incident. Some altitude excursions during these maneuvers due to nervousness, passenger distrs and IMC/turbulence conditions. Notes: ATC performed admirably when faced with nervous pilot. Smoothed entire process to allow safe handling, comfortable outcome. This pilot learned much about: 1) CRM. 2) extra pressures imposed by intimidation of carrying passenger while dealing with busy, unfamiliar airspace.

Title: PA28 PLT MADE MULTIPLE APCHS AND MISSED APCHS WITH TRACK AND ALTDEVS PRIOR TO SAFE LNDG AT LUK.
Narrative: IFR FLT PLAN TO CINCINNATI AREA. HAD PLATE OUT FOR HAMILTON ARPT, LOC APCH, BUT LOC OTS. RECEIVED AUTH FOR NDB APCH VECTORS. COULD NOT PRODUCE PLATES IN TIME, LOST IN COCKPIT. MISSED APCH. ATC VECTORED TO 2600 FT MINIMUM BUT UNABLE TO SEE ARPT. RECEIVED VECTORS TO BACK COURSE APCH AT LUK, BUT MISSED. MESSED UP APCH AGAIN DUE TO FAILURE TO PRODUCE PLATES FROM COCKPIT. EXECUTED MISSED APCH AND RECEIVED VECTORS FOR ILS TO OPPOSITE END OF SAME RWY. NO FURTHER DIFFICULTIES. LANDED WITHOUT INCIDENT. SOME ALT EXCURSIONS DURING THESE MANEUVERS DUE TO NERVOUSNESS, PAX DISTRS AND IMC/TURB CONDITIONS. NOTES: ATC PERFORMED ADMIRABLY WHEN FACED WITH NERVOUS PLT. SMOOTHED ENTIRE PROCESS TO ALLOW SAFE HANDLING, COMFORTABLE OUTCOME. THIS PLT LEARNED MUCH ABOUT: 1) CRM. 2) EXTRA PRESSURES IMPOSED BY INTIMIDATION OF CARRYING PAX WHILE DEALING WITH BUSY, UNFAMILIAR AIRSPACE.
