Narrative:
After takeoff from slc, departure control gave us a 110 degree heading to intercept slc 094 degree radial eastbound. When we were xferred to ZLC frequency, I asked the controller if we could proceed RNAV on course. She approved our request and we made a southwest turn to our next filed fix which was hve. The controller asked us if our filed route of slc 094 degree radial to mtu was given to us by slc departure control. I said 'negative,' that our pre departure clearance showed direct to hve-dvc-lvs, etc. She then cleared us direct to ALS-cim-txo flight planned route. After re-examining our printed pre departure clearance, I noticed that I had mistakenly read our original routing of slc FFU3 hve dvc lvs...iah, which was printed below the proper ATC clearance of SLC094 MTU303 mtu hbu ALS cim txo...iah.

Title: ACR CREW DEVIATES FROM CLRED RTE AFTER MISREADING PDC IN ZLC AIRSPACE.
Narrative: AFTER TKOF FROM SLC, DEP CTL GAVE US A 110 DEG HDG TO INTERCEPT SLC 094 DEG RADIAL EBOUND. WHEN WE WERE XFERRED TO ZLC FREQ, I ASKED THE CTLR IF WE COULD PROCEED RNAV ON COURSE. SHE APPROVED OUR REQUEST AND WE MADE A SW TURN TO OUR NEXT FILED FIX WHICH WAS HVE. THE CTLR ASKED US IF OUR FILED RTE OF SLC 094 DEG RADIAL TO MTU WAS GIVEN TO US BY SLC DEP CTL. I SAID 'NEGATIVE,' THAT OUR PDC SHOWED DIRECT TO HVE-DVC-LVS, ETC. SHE THEN CLRED US DIRECT TO ALS-CIM-TXO FLT PLANNED RTE. AFTER RE-EXAMINING OUR PRINTED PDC, I NOTICED THAT I HAD MISTAKENLY READ OUR ORIGINAL ROUTING OF SLC FFU3 HVE DVC LVS...IAH, WHICH WAS PRINTED BELOW THE PROPER ATC CLRNC OF SLC094 MTU303 MTU HBU ALS CIM TXO...IAH.
