Narrative:

The company I work for has a reserve policy whereby those on reserve are required to be at work within 1 hour after trip notification. The on-call period is 24 hours a day for as many days as the pilot is on reserve. I was on call for 57 hours when I was notified of a flight that reported 5 hours later. Due to the company's policy, I was unable to comply with far 121.471(B) regarding rest requirements. The far states 'except as provided in paragraph (C) of the section, no certificate holder conducting domestic operations may schedule a flight crew member and no flight crew member may accept an assignment for flight time during the 24 consecutive hours preceding the scheduled completion of any flight segment without a scheduled rest period during that 24 hours....' when I completed the flight segment I had not had a scheduled rest period during the preceding 65 hours.
