Narrative:

Upon review of logbook, found signoff discrepancy of forward entry slide. I also noticed at this time signoff was done by an air carrier employee, which further led me to look to see who the employee was, since there is not any aircraft maintenance at xbx, first officer. I checked to see who the employee was, and as it turns out, it was a flight attendant who signed off the item. Aircraft maintenance management was notified immediately to reassure proper operation of forward entry slide.

Title: AN MD80 WAS DISPATCHED AND OPERATED IN NON COMPLIANCE WITH A FORWARD ENTRANCE DOOR ESCAPE SLIDE DISCREPANCY SIGNED OFF IN THE LOGBOOK BY AN UNQUALIFIED AND UNLICENSED FLT ATTENDANT.

Narrative: UPON REVIEW OF LOGBOOK, FOUND SIGNOFF DISCREPANCY OF FORWARD ENTRY SLIDE. I ALSO NOTICED AT THIS TIME SIGNOFF WAS DONE BY AN ACR EMPLOYEE, WHICH FURTHER LED ME TO LOOK TO SEE WHO THE EMPLOYEE WAS, SINCE THERE IS NOT ANY ACFT MAINT AT XBX, FO. I CHKED TO SEE WHO THE EMPLOYEE WAS, AND AS IT TURNS OUT, IT WAS A FLT ATTENDANT WHO SIGNED OFF THE ITEM. ACFT MAINT MGMNT WAS NOTIFIED IMMEDIATELY TO REASSURE PROPER OP OF FORWARD ENTRY SLIDE.
