Narrative:

Flight attendant #4 came up to first class to tell me that she suspected a young female passenger had been smoking in the aft lavatory. (Not sure which side.) she approached the young woman and asked her if she had been smoking. The passenger said yes, but only a little! Flight attendant #4 found the cigarette butt in the ashtray of the lavatory and came to tell me. As flight attendant #1, I filled out a passenger incident report (smoking incident) and had the passenger complete it with her name, address and drivers license number. (I'm sorry that I do not have a copy of this report.) we explained how dangerous it was to smoke on an aircraft and that there was a huge penalty if she has a second occurrence. The report was filed with airline that evening.

Title: FLT ATTENDANT RPT, MD80, ORD-LAX, PAX SMOKED IN LAVATORY.

Narrative: FLT ATTENDANT #4 CAME UP TO FIRST CLASS TO TELL ME THAT SHE SUSPECTED A YOUNG FEMALE PAX HAD BEEN SMOKING IN THE AFT LAVATORY. (NOT SURE WHICH SIDE.) SHE APCHED THE YOUNG WOMAN AND ASKED HER IF SHE HAD BEEN SMOKING. THE PAX SAID YES, BUT ONLY A LITTLE! FLT ATTENDANT #4 FOUND THE CIGARETTE BUTT IN THE ASHTRAY OF THE LAVATORY AND CAME TO TELL ME. AS FLT ATTENDANT #1, I FILLED OUT A PAX INCIDENT RPT (SMOKING INCIDENT) AND HAD THE PAX COMPLETE IT WITH HER NAME, ADDRESS AND DRIVERS LICENSE NUMBER. (I'M SORRY THAT I DO NOT HAVE A COPY OF THIS RPT.) WE EXPLAINED HOW DANGEROUS IT WAS TO SMOKE ON AN ACFT AND THAT THERE WAS A HUGE PENALTY IF SHE HAS A SECOND OCCURRENCE. THE RPT WAS FILED WITH AIRLINE THAT EVENING.
