Narrative:
While flying between san francisco and narita, japan, we learned that 1 of our passenger (4 months pregnant) was experiencing abdominal pain and tenderness. Approximately 300 mi off the oregon coast, we were dispatched and cleared to seattle, wa. While on HF coverage, we proceeded direct to sedar intersection and declared a medical emergency. In haste to accomplish pre-landing confign from being well overweight, we dumped fuel at FL310 west of sedar intersection and failed to advise ATC. Previous HF communications had been tedious, and in trying to coordinate re-dispatch, ATC clearance, station notification, and descent and landing configns in an abnormal situation resulted in a failure to advise ATC that we were dumping fuel to reach maximum landing weight.
