Narrative:

On the date in question, may/xa/99, on aircraft xyz, I inadvertently missed a completion signature on the second page of work card wxxz while checking the work cards back in. I did not intentionally leave this task unsigned. Upon receipt of this letter of investigation, I contacted production control ZZZ to inquire about work card wxxz the particular task, paragraph 2, item 1, and the mechanic's assigned to that work card. That same mechanic has since signed for the completion of that task. I inadvertently missed an unsigned block and will comply with recommendations.

Narrative: ON THE DATE IN QUESTION, MAY/XA/99, ON ACFT XYZ, I INADVERTENTLY MISSED A COMPLETION SIGNATURE ON THE SECOND PAGE OF WORK CARD WXXZ WHILE CHKING THE WORK CARDS BACK IN. I DID NOT INTENTIONALLY LEAVE THIS TASK UNSIGNED. UPON RECEIPT OF THIS LETTER OF INVESTIGATION, I CONTACTED PRODUCTION CTL ZZZ TO INQUIRE ABOUT WORK CARD WXXZ THE PARTICULAR TASK, PARAGRAPH 2, ITEM 1, AND THE MECH'S ASSIGNED TO THAT WORK CARD. THAT SAME MECH HAS SINCE SIGNED FOR THE COMPLETION OF THAT TASK. I INADVERTENTLY MISSED AN UNSIGNED BLOCK AND WILL COMPLY WITH RECOMMENDATIONS.
