Narrative:
We were cleared to san -- this is what was read back: 'V186 164 degree radial from pom to V208 to 320 degree radial to mzb direct to san.' we were rushed and pressured by tower for takeoff. Advised them this was a 'training flight.' during takeoff roll, 40 KIAS, received altitude change from 4000 ft to descend and maintain 3000 ft. Captain misset departure frequency, called on wrong frequency at 1200 ft AGL, leveled off at 3000 ft runway heading before departure was contacted. Departure asked if tower gave us a heading. We said negative, then tower said turn left to 090 degrees. Immediately we complied. They then wanted to know if we received glendale 7 departure. Captain said yes. However, I never read or received it when I received clearance. Controller advised us there was a departure off of vny. We were handed off and next controller advised us the previous controller called him to relay the same message again. We told him we would report it. This was my first flight from left seat (captain training), no passenger. We departed runway 16R from vny. Copies of SID and clearance are enclosed.

Title: FLC OF A BRITISH HAWKER HS125-800 FAILED TO TURN DURING AN INITIAL CLB PER SID, OR CONTACT DEP AT A LOWER ALT, RESULTING IN BEING VECTORED ON COURSE BY ATC.
Narrative: WE WERE CLRED TO SAN -- THIS IS WHAT WAS READ BACK: 'V186 164 DEG RADIAL FROM POM TO V208 TO 320 DEG RADIAL TO MZB DIRECT TO SAN.' WE WERE RUSHED AND PRESSURED BY TWR FOR TKOF. ADVISED THEM THIS WAS A 'TRAINING FLT.' DURING TKOF ROLL, 40 KIAS, RECEIVED ALT CHANGE FROM 4000 FT TO DSND AND MAINTAIN 3000 FT. CAPT MISSET DEP FREQ, CALLED ON WRONG FREQ AT 1200 FT AGL, LEVELED OFF AT 3000 FT RWY HDG BEFORE DEP WAS CONTACTED. DEP ASKED IF TWR GAVE US A HDG. WE SAID NEGATIVE, THEN TWR SAID TURN L TO 090 DEGS. IMMEDIATELY WE COMPLIED. THEY THEN WANTED TO KNOW IF WE RECEIVED GLENDALE 7 DEP. CAPT SAID YES. HOWEVER, I NEVER READ OR RECEIVED IT WHEN I RECEIVED CLRNC. CTLR ADVISED US THERE WAS A DEP OFF OF VNY. WE WERE HANDED OFF AND NEXT CTLR ADVISED US THE PREVIOUS CTLR CALLED HIM TO RELAY THE SAME MESSAGE AGAIN. WE TOLD HIM WE WOULD RPT IT. THIS WAS MY FIRST FLT FROM L SEAT (CAPT TRAINING), NO PAX. WE DEPARTED RWY 16R FROM VNY. COPIES OF SID AND CLRNC ARE ENCLOSED.
