Narrative:
Failure to switch from approach to tower. We were vectored to final and requested the ILS approach, cleared for the approach and contact the tower over the OM which we failed to do. Contributing factors: displaced threshold, no GS, maintain 170 KTS to the marker, descending from 4000 ft down to 3000 ft to the marker, visibility -- haze, not monitoring copilot's activities, and did not 'verify cleared to land.'

Narrative: FAILURE TO SWITCH FROM APCH TO TWR. WE WERE VECTORED TO FINAL AND REQUESTED THE ILS APCH, CLRED FOR THE APCH AND CONTACT THE TWR OVER THE OM WHICH WE FAILED TO DO.
