Narrative:
I went to my aeromedical physician for my annual second class physical. Everything during the physical went fine and the paperwork seemed ok. However, little did I realize that although I received and paid for a second class examination, the receptionist mistakenly typed class three on the form. The end result was me happily walking away with a useless medical certificate. I started a trip as a first officer with my airline consisting of 4 days of flying and 13 individual flts. On the second to last flight, the crew was receiving a line check from a company standards captain. When he checked my license and medical certificate, he looked at it more thoroughly than I did and saw that it was only a class three. The standards captain was able to displace me and fly the 2 final legs of the trip. I called my physician and he had a corrected medical certificate ready for me when I went to his office the next day. In the future, you can be sure that I will double check every single item on my next medical certificate.

Title: MEDICAL EXAMINER INCORRECTLY FILLED OUT PLT'S CERTIFICATE AS THIRD CLASS. PLT OPERATED ACFT REQUIRING SECOND CLASS AND WAS CAUGHT BY CHK PLT. CORRECTIONS WERE MADE.
Narrative: I WENT TO MY AEROMEDICAL PHYSICIAN FOR MY ANNUAL SECOND CLASS PHYSICAL. EVERYTHING DURING THE PHYSICAL WENT FINE AND THE PAPERWORK SEEMED OK. HOWEVER, LITTLE DID I REALIZE THAT ALTHOUGH I RECEIVED AND PAID FOR A SECOND CLASS EXAMINATION, THE RECEPTIONIST MISTAKENLY TYPED CLASS THREE ON THE FORM. THE END RESULT WAS ME HAPPILY WALKING AWAY WITH A USELESS MEDICAL CERTIFICATE. I STARTED A TRIP AS A FO WITH MY AIRLINE CONSISTING OF 4 DAYS OF FLYING AND 13 INDIVIDUAL FLTS.
