Narrative:
As #1 flight attendant, it was brought to my attention that passenger had asked her seat mate if there was anywhere to smoke on board. Her seat mate happened to be an in-flight service manager. Passenger attempted to get by the middle cabin flight attendants, who were in aisle serving drinks, to use aft lavatory. Since passenger was sitting in first class, I suggested she use first class lavatory, so I could keep an 'eye' on her -- if her intent was on lighting up. Before entering lavatory, she asked me to set 2 glasses of white wine at her seat. Once passenger was inside lavatory, myself and other flight attendant smelled smoke. I knocked on door until passenger opened door. Cigarette pack and lighter in clear view. I asked where she extinguished cigarette. She denied smoking at all, and refused to remove herself from lavatory. I informed captain of situation. Once in her seat she was very disruptive to the entire cabin. I mentioned to her, if her behavior continued I would have security meet the flight. When the service was completed, and seatbelt sign was off, she went to the back of the aircraft. I had informed flight attendant #2 of situation and to keep an eye on her. Passenger again lit up in aft lavatory. Flight attendant #2 opened door and told passenger to return to seat. Security did meet flight. During deplaning, passenger handed me a piece of gum and a napkin, in which she had written that she was very sorry for her behavior. Police instructed her to apologize, in which she did. I said it was a little too late, and I told the customer service manager that she should be denied boarding on her continuation flight to evansville. From what I understand, they reimbursed her for her evansville leg.

Title: FLT ATTENDANT RPT, S80, DFW-ORD, PAX SMOKED IN LAVATORY TWICE, ALCOHOL RELATED. POLICE MET AIRPLANE. PAX BLACKLISTED FROM AIRLINE.
Narrative: AS #1 FLT ATTENDANT, IT WAS BROUGHT TO MY ATTN THAT PAX HAD ASKED HER SEAT MATE IF THERE WAS ANYWHERE TO SMOKE ON BOARD. HER SEAT MATE HAPPENED TO BE AN INFLT SVC MGR. PAX ATTEMPTED TO GET BY THE MIDDLE CABIN FLT ATTENDANTS, WHO WERE IN AISLE SERVING DRINKS, TO USE AFT LAVATORY. SINCE PAX WAS SITTING IN FIRST CLASS, I SUGGESTED SHE USE FIRST CLASS LAVATORY, SO I COULD KEEP AN 'EYE' ON HER -- IF HER INTENT WAS ON LIGHTING UP. WHEN THE SVC WAS COMPLETED, AND SEATBELT SIGN WAS OFF, SHE WENT TO THE BACK OF THE ACFT. I HAD INFORMED FLT ATTENDANT #2 OF SIT AND TO KEEP AN EYE ON HER. PAX AGAIN LIT UP IN AFT LAVATORY. FLT ATTENDANT #2 OPENED DOOR AND TOLD PAX TO RETURN TO SEAT. SECURITY DID MEET FLT. DURING DEPLANING, PAX HANDED ME A PIECE OF GUM AND A NAPKIN, IN WHICH SHE HAD WRITTEN THAT SHE WAS VERY SORRY FOR HER BEHAVIOR. POLICE INSTRUCTED HER TO APOLOGIZE, IN WHICH SHE DID. I SAID IT WAS A LITTLE TOO LATE, AND I TOLD THE CUSTOMER SVC MGR THAT SHE SHOULD BE DENIED BOARDING ON HER CONTINUATION FLT TO EVANSVILLE. FROM WHAT I UNDERSTAND, THEY REIMBURSED HER FOR HER EVANSVILLE LEG.
