Narrative:
In sum: an air carrier left concourse at mia with clearance to runway 9L via taxiway P. Rounding the concourse end they found another aircraft and vehicles blocking the way. The first officer was involved in checklists and understood his captain to say that he knew the way. The captain became confused after negotiating around the blocking traffic and found himself on the active runway 12. They expedited across the runway and called ground for further clearance. Captain's corrective actions: stop until traffic is out of the way and verify position. Captain's contributing factors; confusing signage and marking as well as sea of lights at night. No flashing runway incursion lights for runway 12. Requests improved ground control. First officer's corrective actions: leave SOP's until later in the taxi in order to monitor the taxi more closely.

Title: ACR CROSSES RWY WITHOUT CLRNC AFTER BECOMING CONFUSED DURING TAXI.
Narrative: IN SUM: AN ACR LEFT CONCOURSE AT MIA WITH CLRNC TO RWY 9L VIA TXWY P. ROUNDING THE CONCOURSE END THEY FOUND ANOTHER ACFT AND VEHICLES BLOCKING THE WAY. THE FO WAS INVOLVED IN CHKLISTS AND UNDERSTOOD HIS CAPT TO SAY THAT HE KNEW THE WAY. THE CAPT BECAME CONFUSED AFTER NEGOTIATING AROUND THE BLOCKING TFC AND FOUND HIMSELF ON THE ACTIVE RWY 12. THEY EXPEDITED ACROSS THE RWY AND CALLED GND FOR FURTHER CLRNC. CAPT'S CORRECTIVE ACTIONS: STOP UNTIL TFC IS OUT OF THE WAY AND VERIFY POS. CAPT'S CONTRIBUTING FACTORS; CONFUSING SIGNAGE AND MARKING AS WELL AS SEA OF LIGHTS AT NIGHT. NO FLASHING RWY INCURSION LIGHTS FOR RWY 12. REQUESTS IMPROVED GND CTL.
