Narrative:

I work for an airline which has us do a monthly hour tracking form. Unfortunately, the month ended in the middle of a 7 day flight period. I started a new form but made a mistake counting back days between the present and previous month's form. I, therefore, left one day off and wound up flying 45 mins over the maximum allowed for a 7 day period. I did not discover this until I completed the form for that period. The company schedulers gave me no indication I was approaching my limit.
