Narrative:

After working for more than 11 hours of duty, we started the 6TH leg flight, ILS into mdw. The controller told us to do 180 KTS to the OM and then call tower. Because we were fatigued from a long duty day, we both forgot to call the tower at the OM and I maintained an airspeed of 180 KTS. When we were inside the OM, we remembered to call tower. At about that time we called, we broke out into VFR and we saw an aircraft sitting on the runway we were landing on. Both the tower and I initiated a go around. Contributing factors: too long a duty day! How it was discovered: by luck. Corrective actions: stop the duty and flts both at 8 hours. Flight or duty time. I have never seen any pilot that got any better after 8 hours of duty time. It is always a slow downhill progression of mistakes that lead to sits like this.

AT ABOUT THAT TIME WE CALLED, WE BROKE OUT INTO VFR AND WE SAW AN ACFT SITTING ON THE RWY WE WERE LNDG ON. BOTH THE TWR AND I INITIATED A GAR. CONTRIBUTING FACTORS: TOO LONG A DUTY DAY! HOW IT WAS DISCOVERED: BY LUCK. CORRECTIVE ACTIONS: STOP THE DUTY AND FLTS BOTH AT 8 HRS. FLT OR DUTY TIME. I HAVE NEVER SEEN ANY PLT THAT GOT ANY BETTER AFTER 8 HRS OF DUTY TIME. IT IS ALWAYS A SLOW DOWNHILL PROGRESSION OF MISTAKES THAT LEAD TO SITS LIKE THIS.
