Narrative:

Upon landing on runway 27 at cdg at night and in poor WX due to gusty winds and rain, the aircraft exited the runway at highspd taxiway 11. Tower instructed that contact be made with ATC ground controllers and instructions received were 'taxi spot 1 - india 47.' the crewmembers had the airport diagram open and displayed as required by the company. The txwys were poorly lit and at the intersection of taxiway 11 and taxiway B, the crew saw brightly lit green taxi lights 90 degrees to the right and poorly lit green taxi lights in front of aircraft. Crew chose to turn to the west on taxiway B as indicated and allowed by one way arrows on airport chart. During turn onto taxiway B, right mains and portion of center gear departed hard surface. Number 3 engine was shutdown as a precaution against FOD. A taxiway sign for taxiway 11 was not properly lit to identify position due to non-illustrated number on the sign. Ground controllers and company operations advised of situation. No damage found to aircraft which was prepared for next flight shortly thereafter. Chain of events: gust crosswinds, thunderstorms, end of a long crew duty day. Taxiway signs need to be repaired immediately and burned out bulbs replaced immediately. Many taxiway lights and signs burned out at cdg. Poorly lit txwys throughout airport. Taxi diagram needs to show where wide bodies can turn and not turn based on wide enough fillets to handle main trucks of wide bodies. Company needs to be more proactive in identifying poor fillets at cdg and warning aircrews not to turn on these inadequate fillets. Company knew for two weeks prior to this incident that there had been taxi problems into and out of the new ramp at cdg. There had been mud and grass showing up on another dc-10 and another md-11 and the company did nothing to inform flight management, flight safety, flight standards or flight training to warn other aircrews of dangers of taxiway fillets not being stressed to handle wide bodies. Human performance considerations: crews need better opportunity to get quality rest not just opportunity to sleep but chance to get restful sleep based on circadian rythm.

TXWY SIGNS NEED TO BE REPAIRED IMMEDIATELY AND BURNED OUT BULBS REPLACED IMMEDIATELY. MANY TXWY LIGHTS AND SIGNS BURNED OUT AT CDG. POORLY LIT TXWYS THROUGHOUT ARPT. TAXI DIAGRAM NEEDS TO SHOW WHERE WIDE BODIES CAN TURN AND NOT TURN BASED ON WIDE ENOUGH FILLETS TO HANDLE MAIN TRUCKS OF WIDE BODIES. COMPANY NEEDS TO BE MORE PROACTIVE IN IDENTIFYING POOR FILLETS AT CDG AND WARNING AIRCREWS NOT TO TURN ON THESE INADEQUATE FILLETS. COMPANY KNEW FOR TWO WEEKS PRIOR TO THIS INCIDENT THAT THERE HAD BEEN TAXI PROBLEMS INTO AND OUT OF THE NEW RAMP AT CDG. THERE HAD BEEN MUD AND GRASS SHOWING UP ON ANOTHER DC-10 AND ANOTHER MD-11 AND THE COMPANY DID NOTHING TO INFORM FLT MANAGEMENT, FLT SAFETY, FLT STANDARDS OR FLT TRAINING TO WARN OTHER AIRCREWS OF DANGERS OF TXWY FILLETS NOT BEING STRESSED TO HANDLE WIDE BODIES. HUMAN PERFORMANCE CONSIDERATIONS: CREWS NEED BETTER OPPORTUNITY TO GET QUALITY REST NOT JUST OPPORTUNITY TO SLEEP BUT CHANCE TO GET RESTFUL SLEEP BASED ON CIRCADIAN RYTHM.
